
What do people see in their final dreams before death? A new study suggests that many terminally ill patients share strikingly similar visions in their last days, offering a glimpse into the mysterious transition from life to whatever comes next.
Researchers from Azienda USL–IRCCS di Reggio Emilia in Italy surveyed 239 hospice workers, nurses, and psychologists about the end-of-life dreams and visions, or ELDVs, reported by their patients. The findings, published in the journal Death Studies, reveal that certain themes and images tend to surface as death approaches.
According to the study, many patients described dreams of reuniting with deceased loved ones. One professional recalled a patient who dreamed of her husband saying, “I’m waiting for you,” which was seen as a sign of inner peace and acceptance.
Others reported visions filled with symbols of transition, like bright light, doorways, and staircases. One patient remembered climbing barefoot toward an open door filled with light. Another saw a white horse galloping along a shoreline, a serene image that brought comfort.
But not all dreams were peaceful. Some patients experienced disturbing visions, such as a monster with the face of their mother dragging them down. Researchers believe these distressing dreams may reflect unresolved conflict or a fear of letting go.

The team notes that talking about these dreams can help patients approach difficult topics without triggering denial. “ELDVs carry an important relational potential,” they explained, allowing patients to express themselves through symbolic imagery.
This study is the first to focus specifically on the sleep visions of the terminally ill, rather than those who have had near-death experiences. Despite their prevalence, many patients hesitate to share these dreams, fearing ridicule or judgment.
So what do these visions really mean? Researchers say the reason remains unclear, but they warrant deeper exploration. Could these dreams be a natural part of the mind’s way of coping with the end of life?
